Cancelling OSHC coverage that has already started

When the OSHC coverage period being cancelled has already started, the form requires a signed insurance authority form and a confirmation that the student's international movement records support the cancellation. The form locks the coverage-status answer when the dates show coverage has begun.
When the OSHC coverage period you are cancelling has already started, the form asks for two things in addition to the documents for your cancellation reason.
1. Signed insurance authority form
Upload the student's signed insurance authority form. Use the provider's download link when the form offers one.
Follow the instruction on the form: the student's signature only. Leave the other fields blank.
Authority forms accept files up to 20 MB, rather than the 10 MB limit that applies to other documents.
2. International movement records confirmation
Check the student's international movement records, then select the confirmation that they support the cancellation.
The coverage dates decide this, not your answer
If the coverage dates show that the selected period has already started, the form locks that answer and requires the started-policy documents.
For an amendment, check the amendment's own coverage period rather than assuming it has the same dates as the base policy.
