How to create a form

Forms collect information from new and existing students — lead capture for new contacts, data collection for students already in your pipeline. Build a form by dragging fields, then configure how submissions are handled in Form Settings. Submissions auto-fill or update the student profile when standard student fields are used.
Forms in Edvisor capture information from new and existing students. New leads who fill in a form become student profiles in your pipeline. Existing students who fill in a form update their own profile. Forms live in Settings → Recruitment → Forms.
Create a new form
- Go to Settings → Recruitment → Forms.
- Click + Create New Form. The form builder opens with two tabs: Build Form and Form Settings (and Responses once submissions come in).
Build Form — pick the fields
The Form Elements panel on the left has three categories. Drag any element into the form area on the right; reorder by dragging.
- Student — 20 standard student-profile fields: First Name, Last Name, Email, Preferred Name, Gender, Phone Number, Date Of Birth, Home Address, Nationality, Current Location, Passport Number, Visa Type, Secondary Contact, Desired Destination, Desired Schools, Desired Programs, Desired Start Date, Desired Duration Of Study, Accommodation Preference, Budget. When a form using these fields is submitted, Edvisor auto-fills (for new students) or updates (for existing students) the matching field on the student profile.
- Basic — Text Field, Email, Text Area, Url, Number, Phone Number, Address, Date / Time, Day, Checkbox, Time, Select Boxes, Select, Radio, File, and Button. Use these for any data you want to collect that doesn't map to a student profile field.
- Premium — Signature.
Use Preview Form in the top right to see what the student will see at any point.
Form Settings — configure how the form behaves
Information
- Name (required) — appears in the Forms list and in the form URL.
- Post Submission Message — text shown to the user after they successfully submit. Use this for a thank-you message or instructions for next steps.
New Students
When a submission comes from someone whose email isn't already on a student profile, Edvisor creates a new student. These four dropdowns control where the new student lands:
- Office for new students created from form — which office the student belongs to.
- Default pipeline for new students — which pipeline they enter.
- Default pipeline status — which status within that pipeline.
- Assign new students to — which team member becomes the assigned counsellor.
Existing Students
When a submission's email matches an existing student profile and the form contains values for fields that already have data, If field value already exist, what should happen? controls the behaviour:
- Append Value — keep the existing value and add the new one (for fields that hold multiple values).
- Replace Value — overwrite the existing value with the new one.
- Skip — ignore the new value, keep what's already on the profile.
The setting applies to the whole form, not per field.
Notifications
Notification when form is submitted controls who gets emailed when a submission lands:
- Team member assigned to student — only the counsellor assigned to that student.
- Agency admin — admin users in your account.
- All team members — everyone on the team.
Save
Click Save in the top right when you're done. The form is now ready to share — the public URL and embed snippet are available in the Share Form section just below.
